The Role:

• Supervise and support a dedicated team of 6 RN Case Managers

• Act as a mentor and resource, not a micromanager

• Lead with calm, confidence, and emotional intelligence

• Contribute to education, process improvement & case consultations

• Help grow relationships with local SNFs, ALFs & hospitals

• Provide light on-call support

What We’re Looking For:

RN License (Required)
Home Health or Home Care Case Management experience
