The Opportunity

You will serve as the top operational and clinical leader, partnering directly with ownership to scale the business.

Your impact:

• Own day-to-day operations across AFC, respite, and adult day

• Lead and develop clinical teams (RNs, case managers, coordinators)

• Drive census growth (target 25–50 new members/month)

• Oversee financial performance, KPIs, and payor strategy

• Ensure survey readiness and regulatory excellence

• Help expand into new service lines (home care, PCA, homemaker)

This role blends strategy + execution—you’ll have a seat at the table and control of outcomes.

Who They’re Looking For

• RN (BSN or MSN required)

• Proven leader in home health, hospice, long-term care, or community-based care

• Experience with multi-site operations, compliance, and growth

• Someone who thrives in entrepreneurial, scaling environments
