The Role:
As Vice President of Rehabilitation & Case Mix, you will play a key leadership role in driving both clinical and financial performance across the organization. You’ll oversee rehabilitation services and Case Mix strategy, ensuring optimal outcomes, strong PDPM performance, and regulatory compliance.

Key Responsibilities:

• Provide strategic oversight of rehabilitation programs across multiple facilities

• Drive Case Mix Index (CMI) performance and PDPM optimization

• Partner with clinical, MDS, and operations teams to improve quality and reimbursement outcomes

• Support survey readiness and regulatory compliance efforts

• Identify opportunities for program development and growth, efficiency, and clinical excellence

• Lead and develop rehab and MDS teams across the region

What We’re Looking For:

• Multi-site leadership experience in skilled nursing centers

• Strong background in rehabilitation services and Case Mix/PDPM

• Proven ability to improve CMI and overall facility performance

• Hands-on, collaborative leadership style

• Licensed therapist (PT, OT, or SLP)
